for best results, we recommend using a laptop or desktop computer.

Parent information -- work and cell phone numbers, email addresses.
Student information -- demographic and health/medication information.
Emergency contact -- phone numbers.

Child's Birth Certificate
Proof of Residency (Utility Bill, Mortgage/Deed/Title, Lease/Rental Agreement, Recent Financial Statements (paystubs, credit card, tax or bank statements))
Immunization Records / additional Medical Documentation (if applicable)
IEP Documents (if applicable)
